An investigation into the death of an employee at Virginia Glass Products’ Ridgeway, Va., facility in April 2019 has closed with a general duty citation and an $8,271.90 penalty. The case closed on October 17 and the penalty was paid on November 1.

According to Jennifer Rose, Virginia Occupational Safety and Health consultation director at the Virginia Department of Labor and Industry, which carried out the investigation, the company was asked to make corrections as part of the citation. She said the company has since made the corrections, including engineering parts to be added to the equipment involved in the accident, updating procedures and providing training to employees.

Several hundred pounds of glass fell upon the employee while he was loading glass onto a truck, according to the Associated Press.

Virginia Glass Products declined to comment on this update.
